Your First Office visit
As a new patient, we would like you to complete a short but thorough health questionaire. This will help us to provide you with the best possible care. A single upper and lower jaw x-ray (panoramic view) may be taken. Appropriate additional radiographic and diagnostic studies will be performed as needed.

Should you have any questions after your procedure was performed, or need to get in touch with our offices after hours, please call us at ( 904 ) 296 - 2226 or ( 904 ) 797 - 7111. Our patients outside the Duval and St. Johns County calling areas may use our toll free number, (800) 379 - 3557. Your call will be returned as soon as possible.

Call Ahead for Delays
We know how annoying it is to be delayed when you arrive promptly for your appointment. We try our best to maintain our schedule, but sometimes there are unavoidable delays or emergencies.
We ask your patience and suggest that you call our office about 60 minutes before your appointment to see if there will be a delay.

Rescheduling
If you are unable to keep your appointment, we ask that you call us as soon as possible. With enough notice, we may be able to contact another patient who has asked to be called if there is an available opening.

Financial Policy
On most occasions, fees for professional services are due when treatment is provided. All major credit cards, cash and debit cards are accepted. Other financial and payment options are available in addition to conventional insurance, and can be tailored to meet each patient's needs.
Patient coverage and co-payment can generally be provided prior to initial visit when insurance information is given at the time of scheduling. -
Future Health Care
We strongly believe in preventive dentistry and preventive medicine to help avoid future problems. If you are new in the area, or have not been to a dentist or physician in some time, we will be happy to recommend a family dentist or physician to help you achieve and maintain optimum oral and general health.
