As a new patient, we would like you to complete a short but thorough health questionaire. This will help us to provide you with the best possible care. A single upper and lower jaw x-ray (panoramic view) may be taken. Appropriate additional radiographic and diagnostic studies will be performed as needed.
Dr. Wells will personally review your situation with you, discussing treatment options and the advantages and disadvantages of each. We want you to be completely informed and to participate in decisions about your care.

We strongly believe in preventive dentisty and preventive medicine to help avoid future problems. If you are new in the area, or have not been to a dentist or physician in some time, we would be happy to recommend a family dentist or physician to help you achieve and maintain optimum oral and general health.
